Objectives: To evaluate, using cone beam tomography, prevalence, bilateral and features of C-shaped root canal system in mandibular second molars of patients attending the department of oral and maxillofacial radiology at the Universidad Peruana Cayetano Heredia 2011- 2013. Material and methods: 227 seconds mandibular molars were evaluated. The prevalence and bilateral nature of the C-shaped root canal according to sex and location of the tooth were determined and analyzed with Chi-square test. The shape of the C canal are categorized based on the classification of Melton. Results: The prevalence of C-shaped canals was 40.5%, this prevalence is no different with respect to the location of the tooth. The shape of the C canal in the coronal and middle thirds was more common for type III and type I in the apical thirds. The bilateral prevalence was 76.6%. The majority (66.3%) of the second mandibular molars with C-shaped canals, had separated holes and 33.7% had complete C-shaped holes. Conclusions: The prevalence and bilateral of C-shaped canals is high. Most C-shaped canals change its shape along the root.
