Clinical, historical and research aspects of the phenomenon known as “fatigue” are examined. A summary of the main findings regarding its history, epidemiological data, neuroanatomical and psychopathological descriptions, is presented. The correlations between the different types of structural and functional damage are numerous, which generates doubts about the symptom’s existence and forces a redefinition. The search of a physio-pathological mechanisms and therapeutic targets are also evidence of the interest of investigators. Fatigue is cause of significant impact on global health due to the resulting dysfunctionality; such is the reason for the suggestions of using psychometric and factorial analyses and a categorization of its known components in order to leave behind all the confusing elements previously found. It is necessary that future studies, after defining its underlying processes, focus on identifying biological markers and micro- and macroscopic structural damages, so that the quality of life of affected populations can be improved.
