Objective: To show the technique and results of the use of a cosmetic incision in surgical procedures (i.e.: heart surgery) that require a median sternotomy, trying to reduce the psychological problems on young women who need this kind of surgery, by avoiding the use of the regular vertical incision that leaves a more notorius scar. Material and Methods: We present six cases of bilateral submammary cosmetic incision done in young women who had to follow cardiac and/or mediastinal surgical procedures through a median sternotomy with follow up of up to three years. Results: In all the cases we obtained optimal surgical exposure. All the wounds healed well. There were no hematomas. There was a recovery of the initial hyposthesia and the sternum healed well. The cosmetic results were at the entire satisfaction of all patients. Conclusions: The bilateral submammary insicion for median sternotomy offers excelent surgical exposure of the mediastinum and heart. It can be done in our media without additional risks and obtains notorious psychological benefits in female patients. ( Rev Med Hered 1998; 9:10-13).
