Objective: To determine the proportion of patients with definitive diagnosis of acute appendicitis who had a delayed intervention at the Hospital de Apoyo de Chachapoyas (HACH) in a 5-year period and to describe the principal causes of delayed intervention. Material and methods: The Hospital records of patients undergoing appendectomy in HACH during 5-year period were reviewed. It were included all patients with definitive diagnosis of acute appendicitis. Results: 108 patients were included. 41 of them (38%) had a delayed intervention. 19 patients (47%) underwent a delayed operation because to factors controlled by themselves and in 22 patients (53%) the delay was atributted to health services factors. 34 of 41 patients (83%) who had a delayed operation suffered complicated acute appendicitis (CAA). The hospitalization time of patients with CAA was longer (13.21 days) compared with patients with no CAA (3.94 days, p=0.000). Conclusions: There is a high percentage of patients who had a delay of 72 hours or more from initial symptoms and surgery. That was due mainly to delay on looking for attention by the patients and time in arriving to health services because of geographical reasons. Patients who had a delay of 72 hours or more, had complications 2.5 times more frequently than those patients with early surgery. ( Rev Med Hered 2001; 12: 3-7).
