Medical audit, a quality assurance tool, attempts to identify irregularities and dysfunction with the aim to offer alternatives to improve quality. Objective: To identify and analyse the causes and features of Temporary Disability to Work Certificates (TDWC) emissions. Material and Methods: This study was carried out in 17 EsSalud medical centres from Lima and Callao in 1996. A total of 41,918 TDWC given were audit, preferred in a concurrent way, as possible. We excluded those given for antenatal and postnatal rest and those which belonged to "Home Health Care Programme" (PAAD). Eight criteria were established to determine the adequacy of emission. Results: From the total TDWC audited 52.93% had observations, most of them of the administrative type. The most frequent criteria for a TDWC observed were "TDWC not registered in the medical record" (45.46%), "Medical consultation not registered in the medical record" (16.61%) and "No concordance of rest days between medical record and TDWC" (10.82%). Conclusions: Most of the criteria for a TDWC observed were administrative related, usually because of inappropriate filling of medical records and TDWC. We recommend the implementation of a self-sustainable system of concurrent audit and an educational and training programme. (Rev Med Hered 2002; 13: 26-31).
