Objective: An open, randomized clinical trial was performed in order to determine the efficacy of local application of granulate Sugar (GS) vs. 10% yodopovidone (YP) solution in wound infection prevention in patients with secondary peritonitis due to acute appendicitis (SPAA). Methods:: Between 1991 to 1996 a total of 135 patients with SPAA were surgically treated in the Hospital Nacional Cayetano Heredia. Patients were assigned randomly to receive GS o YP in the surgical wound. Results: Groups were comparable in the baseline for gender, age, disease severity, time duration of surgical procedures. The rate of wound infection up to Post-operatory day 5 was 39.39 % (26 / 66) for YP group vs. 28.99% (20/69) (p = 0.20) for CS group. Wound delayed closure (WDC) after Post-operatory day 5 was performed in 89 patients. Incidence of Wound infection in patients after WDC was 40% (16/40) for YP group vs 20.41 % (10/49) for GS Group (p =0.04, OR: 2.6 CI95%= 0.92 a 7.42). Wound infection rate at the end of follow up was 63.64% (42/66) for YP group vs 43.48 % (30 / 69 ) for GS (p= 0.02, OR: 2.28 , IC95% = 1.08 - 4.83). ( Rev Med Hered 2003; 14:74-80 ).
