Rev Med Hered, ene./mar. 2004, vol.15, no.1, p.37-43. ISSN 1018-130X. Objective: To evaluate the sensitivity of the semen culture in the diagnosis of chronic bacterial prostatitis (CBP). Material and Methods: This is a prospective, analytic case series study performed in males with clinical manifestations suggesting CBP and without previous treatment. We evaluated clinical, demographic and laboratory variables. Meares and Stamey test and the test we denominated Alternate (semen culture and 3 urine cultures) were performed in all patients. Sperm culture sensitivity was evaluated. Results: 69/130 patients carried out both tests. The media age was 37.07± 11.16 years old. The media illness time before the physician office visit was 12.5 months. The most frequent symptom was pain in the testes which was present in 32 patients. The digital rectal examination was normal in 64 (92.75%) patients . Alternate test was positive in 7 (10.14%) cases, Escherichia coli was the most frequent organism identified in the sperm culture. Meares and Stamey test was positive in all patients and Staphylococcus aureus was most frequent organism detected in the expressed prostatic secretion culture. The sperm culture sensitivity was 10.14%. Conclusion: In our study, the sperm culture had a low sensitivity in the diagnosis of CBP and its employment could originate the subdiagnosis of this entity.
