Objectives: To determine the utility of the solid unreamed intramedullary nail (UTN) in the treatment of open and close shaft tibial fractures and also in nounions. Materials and methods: This is a prospective and longitudinal study in 12 patients with shaft tibial fracture treated in the orthopedics service at the Hospital Nacional Cayetano Heredia between February 2000 and June 2001. The technique used was the one described in the Manual of Surgical Techniques of the AO. The time of follow up was one (1) year. Results: The mean time of consolidation with this nail was 16 weeks, with no cases of nounions. Nevertheless our patients presented with soft tissue and skin lesions, we didn't observed neither superficials nor deep infections. Also the movements of the knee, ankle and subtalar articulations started in the immediate postoperative period, getting our patients normal articulations ranges. Conclusions: The unreamed intramedullary nail is a good surgical alternative for the open and close grade I fractures of the tibial shaft, also for the nounions, because of all the advantages and good results obtain.
