The accepted standard method for titration actually accepted is the conventional polysomnography. It consumes time, qualified personnel and has a high operational cost. Objective: To evaluate the auto adjustable CPAP use for CPAP titration in patients with diagnosis of moderate to severe Obstructive Sleep Apnea (OSA). Material and methods: We report 54 patients with the diagnosis of OSA done by polysomnography (6) and respiratory polygraphy (48). We used the Autoset T (Resmed®) autoadjustable system for titration. All patients received during an office interview the instructions about how to use the equipment before they were taken it home. If results of the first test were not satisfactory, the patient would take it home for a second test. Results: 54 studies were done, one for each patient. In the first test, 8 studies (15%) failed, so they took the equipment home once again. Finally, only in 2 patients (3.7%) the titration failed and they needed a conventional titration with polysomnography. The final pressure to be programmed to the CPAP was defined according to the graphic’s visual reading. At the moment we finished the data base, 44 (81%) patients were using the CPAP regularly. Conclusions: We consider that non-conventional titration based on the visual reading of auto adjustable CPAP records is an useful, simple, very cheap CPAP titration method in our country.
