Chlorfenapyr pyrrole derivative is a pesticide that uncouples oxidative phosphorylation leading to cell death. The case of intoxication with chlorfenapyr following a suicide attempt is presented. The patient was seen after 24 hours of exposure presenting with abdominal pain, diarrhea and sweating. He developed fever and tachypnea, ancillary tests showed lactic acidosis, azotemia, increased transaminases and prolongation of the prothrombin time. The clinical evolution was torpid with further development of acute renal failure, and disturbance of consciousness. The patient died 42 hours after ingesting the poison. The management of these intoxications is supportive and with gastrointestinal decontamination. There is no antidote to date
