We present the case of a 60-year-old male patient anticoagulated with warfarin who presented with lower gastrointestinal bleeding and hemodynamic instability due to ischemic colitis with high INR after being surgically intervened with Bentall´s procedure and aortic valve replacement with a metallic prosthesis. Treatment with prothrombin complex concentrate was started with good response.
