Brugada syndrome has been historically related to sudden death. However, the diagnosis and estimation of risk may be inaccurate based on cardio vascular risk factors, comorbidities and unexpected clinical evolution. We present the case of a 72-year old male patient admitted to the emergency room with an EKG suggestive of the Brugada pattern type I and with syncope after receiving propafenone. We review the controversies regarding the use of implantable cardiac defibrillator
