Recurrent abdominal pain (RAP) is one of the most frequent queries in pediatrics and most cases have a non-organic cause. RAP can cause significant anxiety in parents and carers, who can become overwhelmed by the pain and are unable to relieve their child’s pain. It is crucial to the realization of a complete history, supported by analytical and radiological tests, being exploratory laparoscopy the last step. Two patients are reported with RAP in right iliac fossa, in which pathologic examination of the appendix revealed as a possible etiology of pain a new entity not well known today: the appendicular fibrosis. Patients with RAP in the right abdominal side, laparoscopic appendectomy could be diagnostic and therapeutic. The study histopathology of the appendix can help define the etiology of the pain, allowing a better understanding of the possible etiologic agents of the RAP
