Abstract

DRASSINOWER H., Enrique. Reacción constitucional producida por el extracto de polvo de casa. Rev. perú. med. exp. salud publica [online]. 1951, vol.8, n.1-4, pp.194-214. ISSN 1726-4634.

1° A general exposition of the house dust has been made, showing that it belongs to the non-seasonal inhalants. It has been remarked that in our Country, house dust is the most important inhalant and the most frequent responsible of the allergic diseases of the respiratory tract. 2° House dust's composition has been exposed and it was remarked that its antigenic activity does not depend on the inhalants that compose it, but upon a specific factor highly antigenic of unknown nature. 3° We believe that house dust actually possesses an antigenic action, and that this activity is due to a specific factor. 4° It was mentioned that there are many prooves in favour of these two principles (one of the most important is the PRAUSNITZ-KÜSTNERS method), but only personal observations are exposed: The good clinical results that have been obtained in our country during the desensitization treatment with autogens and stock or mixed house dust extracts. During the comparative study made with house dust extracts from New York City and autogen and stock house dust extracts from our city, the same clinical results (diagnosis as well as treatment) were obtained. This suggests that the antigenic activity of house dust depends upon a specific factor that might be present in the three extracts. Finally, it was exposed that a definite proove in favour of he antigenic activity of house dust, is the constitutional reaction produced by its extract. It was remarked that these kind of reactions with house dust extract are very unusual. 5° Two clinical cases of constitutional reactions produced with house dust extracts during desensitization treatment are reported. 6° The circunstances of the appearing of the constitutional reactions and the dosis with which they were produced in both cases, are completely different and even opposed. In the first case the constitutional reaction was produced in the beginning of the treatment and with a very low dosis: 0.15 cc. of the 1/10 solution of house dust extract. The highest dosis tolerated was 0.14 cc. from the same solution. This is a case of constitutional reaction due to an extremely hypersensitiveness to house dust. In the second case the constitutional reaction was produced with a higher dosis (0.23 cc. and 0.25 cc. of the concentrated solution) and under the circunstances that we were getting to the maintaining dosis. The highest dosis tolerated was 0.2 cc. of the concentrated solution. This is a case of constitutional reaction due to a high maintaining dosis. 7° In both cases a reproduction of the symptomatology of the dominant allergic disease (bronchial asthma) was observed during the constitutional reactions. 8° In both cases it was possible to produce constitutional reactions with autogen house dust extracts as well as with stock extracts. 9° These are the first cases reported in our country of constitutional reactions produced with house dust extracts, and we are considering this as a contribution in favour of the antigenic activity of house dust.
