Mujer de 61 años, con diagnóstico de diabetes mellitus tipo 2, ingresó por dolor abdominal en flanco izquierdo de cuatro semanas de evolución, vómito post prandial, pérdida del apetito y ausencia de evacuaciones, no refería fiebre. A la exploración física se encontró tumoración de 15x13 cm localizado en mesogastrio y flanco izquierdo, móvil de consistencia firme, de borde regular y doloroso a la palpación. En la tomografía computada se observó tumoración homogénea redondeada con densidad de partes blandas que envuelve vasos y grasa mesentérica, formando el “signo del sándwich” (flecha). Se intervino quirúrgicamente, realizándose una exéresis de tumoración mesentérica con resección intestinal de 90 cm aproximadamente de intestino delgado. El estudio histopatológico con inmunohistoquímica dio como diagnóstico de linfoma de células grandes B, de tipo centroblástico.
A 61-year-old woman with type 2 diabetes was admitted with a four-week history of left flank pain, post prandial emesis, loss of appetite and constipation, no fever was noted. The physical examination revealed a firm and regular tumor of 15 per 13 cm located in the mesogastrium and left flank. The CT scan revealed a rounded homogenous tumor that involved blood vessels and mesenteric fat tissue forming the sandwich sign. The tumor was resected surgically including a 90 cm resection if the small intestine. Immunohistochemistry revealed a centroblastic large-cell B lymphoma.
